Learning Stick Shift, Please Help?

My biggest issue is getting the car moving. I turn on the car with my brake and clutch depressed. First I take my foot off the brake. Then I take my foot off the clutch slowly, but at some point, it starts jumping/jerking, then try pushing the gas and it gets moving but its still jumpy, im like crap so i hit the clutch n switch it to 2nd… and in 2nd and 3rd I'm much smoother. (Have yet to take it in 4th n 5th). My dad can do it pretty smooth, what's the trick?

Suggestion:

you have to press on the gas and release the clutch at the same time at the same speed like it has to be in unison. it basically take time and once you find the sweet spot you wont forget it and it will become second nature
